From the debug logs could see that the bind password is incorrect. I also tried changing the FQDN to the IP address on profile on the NS, no difference?

Problem Cause Incorrect organizational unit is configured in the LDAP server on NetScaler. Checked the STA's are correct and are showing as "up" on the NS.
